Referrals
Collaborative Care Psychiatry is currently accepting referrals for Neurocognitive Assessments.
Neurocognitive Assessments provide a comprehensive evaluation of cognitive functioning, including attention, memory, executive functioning, learning, and processing speed. These assessments are commonly used to support diagnostic clarification for ADHD, learning differences, cognitive concerns, and to inform treatment planning.

We welcome referrals from primary care providers, mental health clinicians, schools, and other healthcare professionals seeking thorough and reliable cognitive evaluation services for their patients.
Assessment results include detailed clinical findings and recommendations to assist with diagnosis, treatment decisions, academic or workplace accommodations, and ongoing care coordination.​

We would appreciate it if you could fax the patient’s most recent chart note related to the Neurocognitive Assessment referral. This information will be used solely for treatment, care coordination, and continuity of care in accordance with HIPAA regulations.
